These sample documents are intended to be employed in their present form, or with modification for an individual training program, or simply used for reference purposes. The ACGME will be the ultimate arbiter of the individual program's structure and content.

The Heart Rhythm Society welcomes any suggestions or sample documents from  your institution. Comments regarding the competencies or the sample curriculum, forms or checklists may be forward to Education department of the Heart Rhythm Society.

Documents will be reviewed by the Clinical and Research Training Committee prior to being posted.
